Refer someone whose mobility is changing at home.

For home care agencies, social workers, discharge teams and GP practices. We visit the person at home, find out why they are struggling, and write a plan that the family and your team can both work from.

Who to refer

Older people whose mobility or function is declining.

  • Weaker or slower since a hospital stay or an illness
  • A fall, a near miss, or a new fear of falling
  • Struggling with the stairs, getting up from a chair, or getting in and out of bed
  • A recovery after an operation that has stalled
  • Pain or stiffness that has made them stop doing things
  • Finding it harder to keep up their daily routine, or becoming more forgetful
  • A gradual decline in mobility that nobody has looked at properly

Making a referral

Four steps. The family is part of every one.

  1. Step 1 · The referral

    Tell the family about us.

    They can book a free consultation call on this website. Or, if the person agrees, email us and we will ring the family.

  2. Step 2 · The call

    We speak to the family first.

    We find out what has changed and say honestly whether a visit would help. If we are not the right service, we say so and suggest who is.

  3. Step 3 · The visit

    An assessment at home, about an hour.

    Strength, balance and walking, stairs and transfers, falls risk, and what in the house is making daily tasks harder.

  4. Step 4 · The plan

    A written plan within 48 hours.

    In plain English, for the family. With the person’s agreement, we send you a copy too, so it can go into their care plan.

What to know before you refer.

Who visits
An HCPC-registered occupational therapist or physiotherapist, whichever the person needs, with years of NHS experience across emergency care, integrated independence teams and complex neurological rehabilitation.
Where
Home visits across London.
How soon
Usually within 5 working days of the call.
Who pays
The person or their family, privately. The assessment is £140. There is no charge to you or your organisation.
What you get back
With the person’s agreement, a copy of the written plan within 48 hours.
What we don’t do
Urgent or out-of-hours care, personal care, nursing or prescribing. If someone has fallen and is hurt, or is suddenly unwell, call 999 or NHS 111.
